Below is a complete list of forms for both prospective and current students. Please be sure to read and follow the instructions carefully. If you have any questions, please contact the Enrollment Services.

Academic Forgiveness Appeal 

Use this form to request academic forgiveness as outlined in the  Academic Forgiveness Appeal  policy.

Application for Admission

There are two ways to apply. Use one of the following methods to apply for admission to Southeast Technical, even if you have applied for admission to another MsSCU college in the past. Complete the ENTIRE application or the registration process will be delayed.

• Online - Complete and submit this application and payment online
• Request Form - Email or call us to request that an application be mailed to you

Arranged Course

If you need to arrange a course with an instructor, refer to Procedure for Arranged Courses.

Authorization to Release Student Info (Red Wing Form ) (Winona Form )

This form gives consent to Southeast Technical to disclose a student's educational records as specified by the student. Visit the college's Educational Records procedures for more information. To request a copy of your transcript, submit the Transcript Request.

Authorization to Withhold Directory Information (Red Wing Form)(Winona Form)

At Minnesota State College-Southeast Technical the following is considered to be directory information and will be available to the public when you begin your classes unless this form is completed and returned to the Office of the Registrar by the fifth day of your starting semester:

• Students name
• City
• Major field of study
• Participation in officially recognized activities and sports
• Dates of attendance
• Graduation date
• Degrees and awards received

Change of Program Major (Red Wing Form)(Winona Form)

Print and submit this form if you decide to change program majors. For more information, visit the college's Changing Program Majors policy.

Change of Student Information (Red Wing Form)(Winona Form)

Use this form to update your student information. It is very important that your contact information be current so faculty and staff can keep you abreast of current information.

Child Care Grant Program Application (PDF - 120 KB)

This application with instructions can be downloaded from the Minnesota Higher Office of Education and is for assistance with child care expenses. A student needs to have completed the Financial Aid application, and eligible students need to be MN residents not receiving MFIP.

Company Billing Authorization (Credit Courses) (Continuing Education)

Complete this form if employer is to be billed for student's tuition, fees, books, or application fee.

Consortium Agreement Southeast Technical's Consortium Agreement (PDF - 72 KB)

All Students who are taking credits at another school who want to receive financial aid for those credits must complete this form. Please print off form, fill out and give to other college for completion.

Course Repeat Notice (Red Wing Form) (Winona Form)

Use this form to notify the college when you repeat a course.

Disability Services Request Form

Submit this form to request a conference to address special needs and to review the available services. It is imperative to Southeast Technical that all students with disabilities have equal opportunity and access to education and training.

Direct Deposit Authorization (2009-10)

August 2009 - August 2010
All students applying for Financial Aid must complete this form as part of the financial aid application process. Included on this form is Direct Deposit information. Any Financial Aid disbursements a student may receive will be directly deposited into the bank account on this form.

Immunization Record

This form is required by the State for all on-campus students.

Intent to Return (Red Wing Form)(Winona Form)

Students returning to Southeast Technical after a lapse of attendance of one semester or more (summer excluded) must submit an Intent to Return form. This form must be submitted at least one week prior to registration.

Minnesota State Grant Program - Student Eligibility Questionnaire -  (2009-10)

August 2009 - August 2010
This worksheet includes additional state grant questions needed to determine a student's state grant award. This worksheet should be completed by students who may be eligible to receive a state grant.

Reciprocity Form A (Red Wing Form)(Winona Form)

If you live in Wisconsin, Iowa, South Dakota, or Manitoba, you are required to complete this reciprocity form.

Reciprocity Form B

If you live in North Dakota you are required to complete this reciprocity form.

Reference Request (Red Wing Form) (Winona Form)

Use this form to request a recommendation from a faculty member or supervisor for a potential work position, scholarship or honorary award and/or admission to another education institution.

Signature Page (2009-10)

August 2009 - August 2010
Students and/or parents who did not sign the original FAFSA form are required to complete this form. The Financial Aid Office will notify you if you are required to complete this form.

Student Grievance Report and Procedure (PDF format)

This is Student Grievance Report and Procedure.

Student Parking Fee Waiver  - Submit Online

If you will not be parking a car in the college lot while a student, submit this waiver.

Student Proctor Agreement Form  - Submit Online

Use this form when your instructor requires proctored testing and you cannot go to the Winona or Red Wing campus to complete these tests.

Substitution of Required Course 

This form is required when there is a need to make a substitution for a required course.

Transcript Request (Red Wing Form) (Winona Form)

Use this form to request a Minnesota State College - Southeast Technical transcript ($2.00/request).

However, if you are applying to a Minnesota State College or University (MNSCU) and attended Southeast Technical since 1990, a transcript request form is not necessary. An electronic transcript will be available at the receiving institution upon application. Inform the college you plan to attend that you should have an eTranscript from Minnesota State College-Southeast Technical.

Verification Worksheet -  Dependent | Independent (2009-10)

August 2009 - August 2010
This worksheet needs to be completed by students who were selected for Verification during the financial aid process. Students who included parent information on the FAFSA should complete the Dependent worksheet. Students who did not complete parent information on the FAFSA should complete the Independent worksheet.
