Student Proctor Agreement Form
The Student Proctor Agreement form is to be used by students who need to take the Accuplacer placement test at a distant location or by students who need to have a test proctored on campus in the Learning Resource Center (LRC) or off campus at an appropriate facility because they are unable to come to campus.
