Personnel/Payroll Information
Completion of the Personnel/Payroll
forms is required for new employees upon hire. These forms provide the human
resources and payroll offices the information needed to enter a new employee
into the data systems. Included in this packet are 1) Personnel & Payroll
Information, 2) Form W-4, 3) Form I-9, 4) Payroll Direct Deposit Authorization,
and 5) Retirement Checklist for New Hires. Wisconsin residents will be provided
a Minnesota-Wisconsin Reciprocity form. Completed forms are returned to
the human resources office on the Red Wing Campus.

Personnel & Payroll Information — provides human resources
with required employee demographic information, identifies previous retirement
plan and post-secondary degrees. Official transcripts are required to verify
degrees for faculty and administrators

W-4 — Internal Revenue Service form that provides payroll with
the tax withholding allowances.
- Form
I-9 — a U.S. Department of Justice Immigration and Naturalization
Service Employment Eligibility Verification for citizenship. Employees need
to provide documents from provided lists.
- Payroll
Direct Deposit Authorization — employees participate in the
direct deposit program with the State of Minnesota. Payroll dollars can
be deposited in one account or multiple accounts on payday. All employees
are required by statute to participate in direct deposit unless a qualified
exception exists.
- Retirement
Checklist for New Hires — provides the human resources office
current retirement information on new employees hired in an unclassified
position.
